Maligni plevralni izlivi - paliativni načini
|
Malignant pleural effusions - palliative methods of the treatmentMalignant pleural effusion is a serious and frequent complication of different carcinomas. It causes pain and irritating cough due to the pressure to the lungs and therefore significantly reduces the quality of the remaining life of the patients. The current practice uses numerous more or less aggressive palliative methods for the treatment of this effusion. The most frequent among them is the chemical pleurodesis with talc; however, it is not efficient enough when the pH value of the malignant pleural effusion is lowered below 7.3 due to carcinomatous or fibrous changes of the pleura (efficacy is defined as a permanent abolishment of the pleural effusion). In recent years the Department for Thoracic Surgery of the Maribor Teaching Hospital has developed a new palliative method in treating malignant pleural effusions - a thoracoscopic mechanical pleurodesis, which turned out to be a safe and successful palliative method, especially at pH values of the malignant pleural effusion below 7.3. Although both - the chemical and mechanical pleurodesis - are equally successful, the chemical pleurodesis with talk represents a more useful palliative method at pH values of the malignant pleural effusion above 7.3 as it is very simple to use. However, in this group of patients, the thoracoscopic mechanical pleurodesis has proved to be an indisputable method, particularly in treating cytologically or histologically uncomfirmed malignant pleural effusions and suspected scarring in the pleural space. |
