Aortoenterične fistuleAortoenterične fistule so komunikacije med prebavno cevjo in aorto. Pojavljajo se lahko spontano pri anevrizmi aorte (primarne fistule) ali po delni zamenjavi aorte z žilno protezo (sekundarne fistule). Značilni simptomi pri bolnikih so krvavitve iz prebavil in bolečine v abdomnu ali križu. Pogoste so tudi septične komplikacije. Pri bolnikih z dokazano anevrizmo abdominalne aorte ali s predhodno rekonstruktivno operacijo na abdominalni aorti in krvavitvijo iz prebavil ali septičnimi komplikacijami je treba v diferencialni diagnozi najprej pomisliti na aortoenterično fistulo in jo dokazati ali izključiti. Spontano nastale fistule lahko zdravimo z resekcijo anevrizme, prešitjem odprtine na črevesju in vstavitvijo nove žilne proteze. Pri fistulah po predhodni rekonstruktivni operaciji na aorti je na voljo več kirurških in znotrajžilnih možnosti. Umrljivost je še vedno velika in se giba med 25% in 90%. Prikazan je primer bolnika s sekundarno aortoenterično fistulo, ki je obilno zakrvavel in je po več kirurških posegih umrl zaradi večorganske odpovedi. |
Aortoenteric fistulaeAortoenteric fistulae are communications between the aorta and the gastro-intestinal tract. They are either spontaneous, resulting from aneurysms of the aorta, or secondary to reconstructive aortic surgery. Patients with aortoenteric fistulas have an overall mortality ranging between 25 and 90 %. Gastrointestinal bleeding associated with abdominal or back pain are probably the most important symptoms to consider. Septic complications may be a frequent symptom in about one third of the patients. Every patient known to have an aortic aneurysm or a history of aortic graft surgery who presents with gastrointestinal bleeding or septic symptoms must be regarded as having an aortoenteric fistula until proved otherwise. Spontaneous fistulae can be treated with resection of the aneurysm, synthetic graft placement and duodenal suturing as the procedure of choice. Fistulae resulting from previous aortic reconstructive surgery can be treated with various surgical or endovascular procedures. A case of a patient with a secondary aorto-enteric fistula and fatal outcome is presented. |
